Registration Procedure:

1. Contact Dorothy Alvarez at 505.321.1083 to hold your spot
2. Download and fill out player forms (youth form for U18 and below). Please make sure forms are complete; Incomplete forms will not be accepted.
3. Provide a picture and proof of age for each player. (Player passcards work well as both age and picture are present)
4. Make check payable to ABQ Futsal in the amount of $525.00 + $10.00 per player for USFF Membership (League insurance included)
5. Contact Dorothy when completed and for drop off information

• Please note that incomplete packets will not be accepted
• WInter schedule will be posted 1 week prior to start of season
